Some lean tissue loss can occur with meaningful weight loss
Diet-induced weight loss can reduce both fat mass and lean mass. Recent evidence suggests that lean-mass loss also occurs during incretin-based obesity treatment. That does not mean GLP-1 therapy uniquely destroys muscle, and the scale cannot tell you how much of a change came from fat, lean tissue, glycogen, or water.
A 2026 meta-analysis reported that the proportion of weight lost as lean mass with incretin therapies was broadly comparable with lifestyle interventions, while lifestyle plus resistance training showed a more favorable lean-mass profile. The useful takeaway is not fear of treatment; it is that muscle-preserving behaviors deserve a place in the plan.
Resistance training is the anchor
Muscle is maintained by giving it a reason to remain useful. Resistance training provides that signal. A systematic review and meta-analysis in adults with overweight or obesity found that adding resistance exercise during dietary weight loss attenuated fat-free-mass loss and improved strength.
The Physical Activity Guidelines for Americans recommend muscle-strengthening activity at least two days per week for adults, alongside aerobic activity. Someone who is new to training does not need an advanced bodybuilding program—the starting point can be simple, progressive, and appropriate to current capacity.
- Train major movement patterns consistently.
- Progress gradually rather than chasing soreness.
- Keep protein and total nutrition adequate enough to support training.
- Track strength, function, and consistency in addition to body weight.
Protein supports the plan; it does not replace the stimulus
Adequate protein can help support lean tissue during energy restriction, but protein alone does not create the same training signal as resistance exercise. The strongest practical strategy combines enough protein, progressive muscle-strengthening work, recovery, and a calorie deficit that does not make the rest of the plan impossible.
If weight is falling rapidly, food intake is very low, strength is dropping sharply, or persistent nausea and poor intake make basic nutrition difficult, that is a reason to involve the prescribing or treating clinician rather than simply adding more supplements.
